E-STATEMENTS AGREEMENT & DISCLOSURES

This Agreement is the contract, which covers your and our rights, and responsibilities concerning Heritage Online and e-statement services offered to you. In this Agreement, the words “you” and “yours” mean those who sign the Membership Application or any Heritage Online authorization form. The words “we,” “us,” and “our” mean the Heritage Trust Federal Credit Union (“Credit Union”). The word “account” means any one or more share accounts you have with the Credit Union.

By completing the following consent form and agreement you are authorizing HERITAGE TRUST FEDERAL CREDIT UNION to deliver statements of your HERITAGE TRUST FEDERAL CREDIT UNION account(s) by electronic means. You understand and agree that by requesting electronic delivery, you WILL NOT receive statements in paper form delivered by regular United States Postal Service mail. You agree that electronic delivery will satisfy HERITAGE TRUST FEDERAL CREDIT UNION'S requirement to provide you a periodic statement of your account(s) activity.

Disclosures

  1. You have a right to receive this disclosure in paper form. To receive a paper disclosure, please contact us by telephone or mail as shown below.
  2. The consent you are providing is for electronic delivery of:
    a. Your periodic HERITAGE TRUST FEDERAL CREDIT UNION account statements,
    b. Your year-end HERITAGE TRUST FEDERAL CREDIT UNION account statement(s).
    and
    c. Your quarterly Advantage Newsletter.
  3. You have the right, at any time and without charge, upon 10-day notice, to withdraw this consent for electronic delivery of your account statements and again receive them by mail. You may withdraw your consent by electronic mail from your e-mail address of record with HERITAGE TRUST FEDERAL CREDIT UNION or by written and signed request.
  4. Even though you have requested electronic delivery of your statements, you may obtain a paper copy of your account statement, at no charge, by utilizing the lobby computers in any of HERITAGE TRUST FEDERAL CREDIT UNION's branches to access your account and print a copy of your statement. You may also receive a copy of your statement by contacting HERITAGE TRUST FEDERAL CREDIT UNION by telephone or mail as shown below and request a copy of your statement at a charge of $6.00.

  5. The software requirements you must meet in order to view your statements electronically are Internet Explorer 5.0 (or higher) or Netscape 5.0 (or higher).
  6. The credit union strongly advises that you save your e-statement to your personal computer or print out a copy of your e-statement for your records. The credit union will keep a 12 month history of your e-statements online at anytime. Requests for copies of older statements may incur a fee for the service.

Request and Agreement for Electronic Delivery of Statements

This is a request to have the statements of your account(s) with HERITAGE TRUST FEDERAL CREDIT UNION delivered to you electronically in accordance with the above consent.

 
 
  1. After your e-statement account is set up, depending on the volume of requests received, we may not be able to set up your account for the first statement cycle following your request.
  2. On or shortly following the first day of each month, (as long as we have a valid email address) you will receive an e-mail message that your e-statement is now available, along with instructions on how you can obtain your e-statement.
  3. Statement notifications can be sent to only one e-mail address. For example, the notification cannot go to both a work and home e-mail address.
  4. It is your responsibility to notify HERITAGE TRUST FEDERAL CREDIT UNION anytime you should have a change in your e-mail address, or if you wish to remove any account from electronic delivery.
  5. If you have more than one HERITAGE TRUST FEDERAL CREDIT UNION account under a separate member number, you need to submit a request for each account.
  6. If you are a joint owner of accounts of other family members, you may elect to also have those statements delivered electronically. You must submit a separate request for each joint owner account.
